This stepped-wedge trial rigorously tests the uptake, likely impact, and costs of a structured care approach to distress screening and management across the NSW and Victorian CIS telephone services. The trial compares the effectiveness of a distress screening model using the DT only (ie, usual care) against a 2-staged distress screening model incorporating the DT, Patient Health Questionnaire (PHQ-4), and a referral model (ie, structured care model).

Evidence for online peer-support forums and email is promising with a RCT of a multicomponent Web intervention reporting decreased global severity scores in a sample of 325 breast and prostate cancer patients [31]. A patient-centered framework is a critical component to improving chronic disease care. Patient-centered care recognizes the values, preferences, and involvement of patients and their loved ones and establishes patients as an expert information source for assessing health care quality [7].
