We used the Heiney-Adams Recruitment Framework to guide our recruitment efforts [40]. The principal investigator mailed a personal letter to potential participants with a colorful and readable frequently asked questions flyer using the STORY logo (designed by focus group participants in STORY). Within 5 days of the mailing, a racially concordant researcher contacted participants by telephone.

A 1-sided P value of .05 will be considered significant for all analyses. I anticipate that the results will support the rationale for an RCT focused on a scalable, multisite, multicounty version of this intervention, with state-level stakeholders such as Select Health, at the conclusion of this study. The University of South Carolina Institutional Review Board reviewed the study protocols for Aims 1 and 2 and these were approved after expedited review.

To recruit participants, we will apply a recruitment model developed by Heiney and colleagues [30], which combine social marketing with relationship building. The PI and a research assistant will work with SCOA and GCCRI staff to identify participants who meet the study eligibility criteria using electronic medical records.

The Healthy Heart app did not significantly increase quality of life (P=.15), but did show clinically relevant changes in heart failure self-care maintenance, management, and confidence. Our preliminary findings are similar to those of Dang et al [33] who identified that an m Health app is a feasible method of assisting health-disparate patients to manage their disease.
