eHealth in Care Coordination for Older Adults Living at Home: Scoping Review
Husebø and Storm [26] reported that the use of video communication in web-based home visits to older adults can facilitate continuous and coordinated care between the patient and health care professionals. Improved information flow among health care professionals in the primary care and specialist health care services can be associated with fewer emergency department (ED) visits and reduce the likelihood of outpatient visits among older adults [27].
